How to get a passport

Ocean County Clerk staff offers assistance to the public in processing U.S. Passport applications at the County Connection, located at the Ocean County Mall, Ocean County Courthouse, Toms River, Northern Resource Center, Lakewood and the Southern Service Center, Manahawkin by appointment. Appointments can be made by clicking the link below:



Please call anyone of our offices if you need help scheduling an appointment, especially if you have imminent travel plans.

All applicants MUST appear in person. Please be on time for your appointment. ALL PASSPORT APPLICATIONS SHOULD BE COMPLETED PRIOR TO YOUR ARRIVAL.
You can visit the U.S. Department of State website to obtain the applications. You can also have the option to pick up the applications at our office BEFORE your appointment but please allow plenty of time to complete your application if you choose to pick up the applications before your scheduled appointment.

PASSPORT BOOK & CARD ARE REAL ID!

The following items are essential to obtain a passport:

  1. Proof of U.S. Citizenship
    • A prior U.S. Passport book or card or a Certified Birth Certificate
      (Note: A Certified Birth Certificate issued by Vital Statistics with a seal and the full names of the parent(s) listed and a recording date within one year of birth.)
    • Born outside the United States – You can obtain a U.S. Passport if you were born outside of the United States. You must submit the following:
      • A previous Passport
      • Certificate of Naturalization or Citizenship
      • Report of Birth Abroad
      • Please contact our office for more information concerning claiming citizenship through US Citizen or Naturalized parent(s) as well as adoption from a foreign country.

  2. Proof of Identification
    • Previous or current U.S. Passport book or card
    • A valid driver’s license with a picture. The license must contain your signature and physical description. Out of State Driver’s Licenses and probationary licenses will require additional identification.
    • Certificate of Naturalization / Citizenship
    • Military Identification
    • Federal, State, County or Municipal Government Employee ID
    • Valid foreign Passport

  3. One Recent 2”x2” Color Passport Photograph (taken within the last 6 months)
    • You can get a passport photo from our office for $10.00.
    • You may bring a passport photo with you but please see the guidelines on the Passport application and keep the following in mind:
      • Photograph must have a light/white background
      • Must be clear
      • Wear normal street clothes (try to avoid white shirts)
      • Do not wear glasses
      • No hair accessories, headgear or hats unless you complete a form explaining that you are wearing for religious or medical reasons.

  4. FEES

    PASSPORT CARD: $15.00
    PASSPORT BOOK & CARD: $115.00
    *EXPEDITE PROCESSING FEE IS $60.00.

    EXECUTION FEE PER APPLICANT: $35.00
    PHOTO FEE PER APPLICANT (If photo is taken in our office): $10.00

  5. U.S. Passport Applications for Children under Age 16 require both parents’ consent.
    • Present evidence that they are the parents or guardians. For Example: Birth Certificate with parental information or legal guardianship papers from the court.
    • Show valid government picture identification. See #2 for acceptable Proof of Identification.
    • Both parents/guardians must appear in person. If one parent/guardian is not available to appear in person, they must submit one of the following:
      • DS-3053 form that is Notarized and has a copy front and back of the non-appearing parent’s identification.
      • Court Custody Order - Original or Certified Copy issued by the Court showing appearing parent has sole or legal custody of the child or a court order from the court allowing the appearing parent the right to make the passport for the child without the other parent’s consent.
      • DS-5525 explaining under penalty of perjury why non-applying parent’s consent cannot be obtained.
      • Certified Death Certificate for a deceased parent.

  6. Children Age 16 - 17
    • Only one parent needs to apply with the child, and they will need to present evidence that they are the Parent/Guardian of the child applying for a Passport. Be aware that if your name does not match the birth certificate which may have been your birth or maiden name or your name does not match the child’s last name, you may need to show proof of your name change (original marriage certificate or original court order for name change).
    • If 16/17-year-old is applying on their own with their own identification, please contact our office for more information.

      *Please note that you will need to know the child’s Social Security number for the application*

PLEASE CALL ONE OF OUR OFFICES IF YOU HAVE ANY SPECIAL CIRCUMSTANCES LIKE ADOPTION, NAME CHANGES, AMENDMENTS TO BIRTH CERTIFICATES OR DO NOT HAVE THE REQUIRED INFORMATION STATED ABOVE. ADDITIONAL DOCUMENTATION MAY BE REQUIRED. ALSO, PLEASE CALL IF YOU NEED AN APPOINTMENT SOONER THAN WHAT IS AVAILABLE ONLINE, ESPECIALLY IF YOU HAVE IMMINENT TRAVEL PLANS.

Birth Certificates & Other Vital Records

In New Jersey, you would contact the Registrar of Vital Statistics in the Municipality where you were born. You can also get your birth certificate and other vital records through VitalChek.
